ASX Accident Report

On the morning of May 10, 2025, a privately operated Piper PA-32RT-300 Lance II, registration N30689, departed from Nassau Airport (83FL) in Yulee, Florida, en route to Fernandina Beach Municipal Airport (KFHB). The sole occupant was a 56-year-old male pilot. Shortly after takeoff at approximately 9:30 a.m. local time, the aircraft began experiencing technical difficulties during its initial climb. Witnesses, including those at the nearby Ron Anderson dealership, observed the aircraft descend abruptly before it impacted a wooded area within a gated residential property near Blackrock Road, approximately 150 meters from the departure runway.

The impact resulted in a substantial fire, with flames reaching the treetops and thick smoke visible from nearby businesses. Surveillance cameras from the dealership captured the aircraft’s descent and the ensuing explosion. First responders, including the local fire department and emergency services, arrived at the scene within minutes. Despite rapid intervention, the aircraft was completely destroyed, and the pilot sustained fatal injuries. There were no passengers onboard and no injuries reported on the ground.

The crash site was located in a narrow wooded area bordering several homes. One nearby resident reported that her property was damaged by fire from the wreckage, stating that the proximity of the crash posed a significant danger to her home. Debris from the aircraft was scattered across the immediate area, further emphasizing the severity of the impact. Environmental conditions at the time included a nearby storm system, with heavy rainfall occurring shortly after the incident. Some speculate the rain may have helped suppress the fire and mitigate the potential for greater property damage.

Preliminary information released by the Federal Aviation Administration (FAA) and National Transportation Safety Board (NTSB) confirmed that the aircraft was en route from Clearwater Airpark and had recently departed Nassau Airport. While eyewitness accounts and visual evidence suggest a rapid loss of control, investigators have not yet determined the exact cause of the crash. The aircraft was reported to be destroyed by both impact and post-impact fire. FAA and NTSB investigators are examining factors such as weather, mechanical integrity, and pilot performance to determine the chain of events that led to the accident.

As of this report, the investigation remains active and ongoing. Authorities have requested that any witnesses or those with relevant video footage contact the Florida Highway Patrol to assist in the inquiry.
